The Community First Choice (CFC)

The Community First Choice (CFC) program offers specific services and assistance to individuals residing in the community who are enrolled in Medicaid and meet the CFC eligibility criteria. These services and supports encompass:

  1. Activities of daily living, such as eating, toileting, and grooming, along with tasks related to independent community living and health-related duties, including personal assistance services.
  2. The acquisition, maintenance, and improvement of skills essential for individuals to care for themselves and live independently in their community, which is referred to as habilitation.
  3. Provision of backup systems or methods to ensure the uninterrupted delivery of services and support, which includes emergency response services.
  4. Training individuals in the selection, management, and dismissal of their own attendants, a component known as support management.

In the state of Texas, CFC may be accessible to Medicaid enrollees, including those benefiting from:

  1. 1915 (c) waiver programs.
  2. Medicaid managed care.
  3. Personal care services for children.

Moreover, individuals can opt for the Consumer Directed Services (CDS) option for specific CFC services.

State plan Medicaid CFC services are available to individuals who require habilitation, personal assistance, or emergency response services and are enrolled in the following Health and Human Services (HHS) waiver programs:

  1. Community Living Assistance and Support Services (CLASS).
  2. Deaf Blind with Multiple Disabilities (DBMD).
  3. Home and Community-based Services (HCS).
  4. Texas Home Living (TxHmL).
